A 78-year-old woman underwent total gastrectomy with Roux-en-Y reconstruction for gastric cancer 5 years and 10 months previously. The cancer was pathologically diagnosed as pT2, N1, P0, H0, stage IIIA. She was hospitalized to our institution this time because she developed difficulty in eating and drinking. Based on a contrast-medium study, we made the diagnosis of reconstructed jejunal stenosis near the Y-anastomosis due to peritoneal dissemination. We decided to treat the stenosis with a self-expandable metallic stent (SEMS). At the time of SEMS placement, we had to use an over-tube in order to keep the device straight. This SEMS placement enabled her to drink water and to ingest liquid food until her death. This is the first report on endoscopic stent dilatation of reconstructed jejunal stenosis in Japan; endoscopy is effective for ameliorating symptoms.
