Purpose: To report the outcome of surgery for idiopathic macular hole with different durations of facedown positioning. Cases and Method: This retrospective study was made on 55 eyes of 54 patients who received surgery for idiopathic macular hole in two institutions during the past 27 months. All the eyes underwent peeling of internal limiting membrane and tamponade with sulfur fluoride. Following surgery, 31 eyes remained in face-down position for a few hours and 24 eyes for a few days. Results: There was no difference between the two groups regarding intraocular pressure on the day after surgery, initial and final rate of hole closure. Iris capture of intraocular lens occurred in 2 eyes that remained in face-down position for a few hours. Conclusion: There was no difference in the rate of success after surgery in two groups that remained in face-down position for a few hours or a few days. Iris capture of intraocular lens is a potential complication after surgery.
