A 64-year-old man was diagnosed with penile cancer and underwent partial penectomy for well-differentiated squamous cell carcinoma, PT1, at his local hospital. He developed enlargement of the right inguinal lymph nodes and was referred to our hospital. Pathological findings of the lymph node biopsy showed well-differentiated squamous cell carcinoma. Combination chemotherapy using cisplatin, methotrexate, and bleomycin was performed and after two courses, he underwent total emasculation, with both inguinal and pelvic lymphadenectomy. One month after surgery, he developed lymphatic edema which induced difficulty in walking, and red papules were observed on his left inguinal region and lower abdomen. A dermatologist in our hospital performed a biopsy, the pathological findings of which revealed squamous cell carcinoma. To our knowledge, metastasis of penile cancer to the skin is very rare.
